4 | EYEWORLD DAILY NEWS | MAY 4, 2019 ASCRS NEWS ASCRS ASOA ANNUAL MEETING alignment, Dr. Maloney said. This technology, he explained, shifts the refractive discussion to postop. You're no longer trying to explain mini-monovision or tolerance with glare and halo preoperatively. Once the patient's refraction stabilizes postop, they then trial and decide what they want their refraction to be, wearing UV light-blocking glasses until their final treatment is complete. Their refraction is set in a 1- to 2-minute in-office treatment with the Light Delivery Device. These are just a few technolo- gies impacting premium refractive cataract surgery and with so many other bells and whistles, what do you really need? This is what Brandon Ayres, MD, Philadel- phia, discussed. Paying attention to the ocular surface is essential, Dr. Ayres said. It impacts biometry and patient satisfaction. In this realm, topog- raphy and tomography can help assess the surface magnitude of astigmatism, he said. Dr. Ayres categorized high-quality biometry with optical biometry as essential as well but with some qualifiers. He said a non-contact biometer is more ac- curate than contact A-scan biom- etry. It also is less user dependent than immersion biometry. Data points, he cautioned, should not be used interchangeably between devices, and there will be cases where optical biometry cannot penetrate the lens to measure axial length. Intraoperative aberrometry he classified as very helpful but not essential. He said this technology can help determine spherical pow- er, axis, and magnitude of astigma- tism. It may be less essential with new IOL formulas and biometers, Dr. Ayres noted. Dr. Ayres considers femto- second lasers for cataract surgery optional for now. While smaller continued from page 1 studies have found advantages to femtosecond laser-assisted cataract surgery, other larger studies have not found it superior to manual cataract surgery. Overall, Dr. Ayres concluded, there is no current technology that can take the place of experience and good clinical decision making and surgical skills. Editors' note: Dr. Waltz has financial interests with Mynosys. Dr. Ianchulev has financial interests with Carl Zeiss Meditec. Dr. Maloney has financial interests with RxSight. Dr. Ayres has financial interests related to his comments. Dr. Ayres gives his perspective on what technology is essential for premium refractive lens surgery.
