Topography is essential to preop planning, said Edward J. Holland, MD, Cincinnati, the symposium's moderator. At the slit lamp, oblique illumination and low magnification will give you the best chance of spotting corneal irregularities, he added. Plan to manage astigmatism at the time of surgery with limbal relaxing incisions (LRIs) for 1.5 D or less of cylinder or a toric IOL for greater than 1.5 D. Patients need to have realistic expectations, Dr. Gupta said, so drive home the mes- sage that refractive stability can take time. X-factor 3: Reducing postoperative inflammation Can surgeons preoperatively predict who will have a postop inflamma- tory response? According to David A. Goldman, MD, Palm Beach Gardens, Fla., to some extent, the answer is yes. Physicians know the common comorbidities and effects that concomitant medications have on inflammation, giving them a precise advantage, he said. There are a variety of offensive and defensive strategies to tackle inflammation. Some surgeons prefer to pretreat with NSAIDs and treat postoperatively with NSAIDs and steroids because they produce a syn- ergistic effect, Dr. Goldman said. His personal regimen is to do no steroid or antibiotic prep, but start NSAIDs 1 day preop. Looking at modern cataract surgery, surgeons are achieving great visual outcomes, and now have to match that success with minimizing postop pain, said Mitchell A. Jackson, MD, Lake Villa, Ill. One way to do that is to reduce the ultrasound energy applied during surgery, he said, using the femtosecond laser. All the femtosecond laser plat- forms show strong reductions in effective phaco time and energy, he said, but most patients still require some level of phaco or irrigation/ aspiration (I/A) to remove the lens material. Dr. Jackson believes that phaco and I/A will evolve over time to compliment laser-assisted surgery, however, which will substantially reduce or even eliminate phaco en- ergy. Not only will this help reduce postop inflammation, but it will also reduce corneal edema and endothelial cell loss, giving the patient a faster recovery and a better experience overall. EW
