EW SHOW DAILY 18 Trifocal IOL update by Lauren Lipuma EyeWorld Contributing Writer A paper session on Saturday afternoon focused on the latest clinical results with trifocal IOLs in Europe. Ruth Lapid-Gortzak, MD, PhD, Driebergen, the Neth- erlands, presented results of a study that compared trifocals with different intermediate focus lengths. She compared outcomes with the AcrySof PanOptix lens (Alcon, Fort Worth, Texas), which has an inter- mediate focus of 60 cm, with the AT LISA tri (Carl Zeiss Meditec, Jena, Germany), which has an intermedi- ate focus of 80 cm. The non-random study of 28 eyes found that both lenses per- formed well at the designed distanc- es and overall, patient satisfaction with the lenses was high. However, Dr. Lapid-Gortzak concluded that the planning of the intermediate power of a trifocal lens should be discussed with the patient. Taller patients will likely benefit from an intermediate focus of 80 cm, while average-height patients will benefit more from a lens that replicates the focus at their own arm length of 60–70 cm, she said. Thomas Kohnen, MD, PhD, Frankfurt, Germany, presented 6-month postoperative results of a multicenter defocus curve eval- uation of the AcrySof PanOptix lens. The prospective, single-arm, non-randomized study determined the mean binocular defocus curve at 6 months postop and described any adverse events. The study found that at 6 months, the binocular defocus curve for the PanOptix lens demonstrated visual acuity of 20/25 or better at near (40 cm), intermediate (60 cm), and far distances. There's a small dip in the curve defocus at the inter- mediate distance for natural vision, which is what Dr. Kohnen observed for the PanOptix lens. Because the defocus curve for this lens is closer to the one for natural vision than for bifocal lenses, this lens gives patients a more natural range of vision, he said. Robert Edward Ang, MD, Ma- nila, Philippines, presented the re- sults of a study comparing tolerance to residual postoperative astigma- tism with small aperture and trifocal IOLs. The study sought to evaluate whether a small aperture could mask the effects of postoperative astigma- tism as compared to trifocal IOLs. Dr. Ang's study compared the IC-8 lens (AcuFocus, Irvine, Cali- fornia), an extended depth of focus lens embedded with a small-aperture mask, with three trifocal IOLs: the FineVision lens (PhysIOL, Liege, Belgium), the AT LISA Tri, and the enVista lens (Bausch + Lomb, Bridge- water, New Jersey). The study found that the small aperture of the IC-8 lens mutes the astigmatism and provides an unin- terrupted extended depth of focus. Regardless of the astigmatism axis, the small aperture lens maintains 20/25 or better vision with up to 1.5 D of induced astigmatism, Dr. Ang said. However, the IC-8 lens did not give patients as good near vision as the trifocal lenses. He concluded that the small aperture is able to mask the effect of postoperative residual astigmatism while also providing patients with an extended range of vision.
