EW SHOW DAILY 30 ASCRS Symposia Tuesday, May 9, 2017 Kendall Donaldson, MD, Plantation, Florida, noted that the MicroSurgical Technology (MST, Redmond, Washington) capsular hooks have been redesigned so they don't have a central hole. "So it is much less likely for the capsular tension ring to go through there," Dr. Donaldson said. Nick Mamalis, MD, Salt Lake City, said the patient should have received the CTR in the initial surgery after zonular problems were identified. Dr. Mamalis generally prefers to use long capsular hooks in such patients because they go into the fornix and hold the capsule for cataract surgery. However, short iris hooks may be preferable when just placing a CTR. "In this setting, using the short- er iris hooks, so long as it doesn't disturb the anterior capsulotomy, is a reasonable way to go," Dr. Mama- lis said. She noted that many surgeons support inserting CTRs with all accommodative lenses, especially when femtosecond laser technology is used. "There's been this debate over whether or not we get a little more fibrotic edge of the capsule, which may induce a little more phimosis, so a lot of people are starting to put capsular tension rings from the outset preemptively," Dr. Donaldson said.
