The retrospective case series of 38 cases, which included use of mitomycin-C in conjunction with the sutureless scleral tunnel and EX-PRESS implant, saw a significant IOP reduction (mean 11.7 mm Hg) in 32 of the 38 patients. Reduction of glaucoma medications postop was an average of 2.3 medications. The scleral tunnel did not appear to impact refractive error, but one pa- tient did experience hypotony with choroidal effusions. When it comes to tube shunt placement, one complication can be encapsulation of the plate, lead- ing to high IOP. H. George Tana- ka, MD, San Francisco, discussed supra-Tenon's implantation of the plate, a limbus-based approach, compared to the traditional in- fra-Tenon's, fornix-based placement. In a retrospective review of Baerveldt (Johnson & Johnson Vision, Santa Ana, California) implant surgeries, 39 were placed infra-Tenon's and 38 supra-Tenon's. While the supra-Ten- on's group had a lower postop IOP —11 mm Hg in the supra group compared to 14 mm Hg in the infra group—Dr. Tanaka said the failure rate was not statistically significant between the two groups (though the supra group was slightly lower). The overall success was slightly higher in the supra-Tenon's group. Paper session included presentations on MIGS, trabeculectomy, and more A Monday morning paper session focused on micro- invasive glaucoma surgery (MIGS), trabeculectomy, and goniotomy, and was moderated by Nathan Radcliffe, MD, New York, with panelists Michael Greenwood, MD, Fargo, North Dakota, and Steven Vold, MD, Fayetteville, Arkansas. One paper discussed efficacy of the collagen implant Ologen (Aeon Astron, Leiden, the Netherlands) in secondary glaucomas after failed trabeculectomy with mitomycin-C. Ologen, explained Heba Magdy Elsaied, MD, Cairo, Egypt, is a bio- degradable, porous, porcine collagen implant. The implant is meant to influence fibrosis by guiding the patterns of fibroblast migration and normalize secreted extracellular matrix deposition. It also helps sep- arate the conjunctiva and episcleral surface, preventing adhesion, Dr. Elsaied said. The study involved 40 eyes with previous failed trabeculec- tomy with medically uncontrolled secondary glaucoma; 18 were open angle glaucoma and 22 had angle closure. Dr. Elsaied said trabeculectomy with the Ologen implant was effec- tive at lowering IOP in patients with failed previous trabeculectomy with mitomycin-C; there was no signifi- cant difference in IOP reduction be- tween patients with secondary open angle and secondary angle closure glaucoma. Based on this data, Dr. El- saied said Ologen can be considered a better alternative to mitomycin-C as a wound modulating agent. Robert Noecker, MD, Fairfield, Connecticut, discussed research that evaluated the safety and efficacy of ab-interno goniotomy with Trab360 (Sight Sciences, Menlo Park, Califor- nia) combined with femtosecond la- ser-assisted cataract surgery. After at least 1 year of follow-up, there was a mean 6.1 mm Hg (31.1%) reduction in IOP and a mean 1.2 reduction in medications from baseline in the combined FLACS-goniotomy cases. These cases did see a need for prolonged use of anti-inflammatory medications, and there was minimal effect on visual acuity, Dr. Noecker said.
