This is a fact he hoped his audience would come to appreciate as he discussed "Preoper- ative Imaging for Cataract Surgery with Premium IOLs" in a Techni- cians & Nurses course sponsored by the Ophthalmic Photographers' Society (OPS). The whole process of preop- erative imaging in preparation for cataract surgery, particularly in view of implanting a premium IOL, is a multistep process that, Dr. Cohen said, "frankly wears everybody out." The process begins with check- in at the clinic, continues through the technician's workup, on to dilation, to taking keratometry measurements using devices such as the IOLMaster (Carl Zeiss Meditec, Jena, Germany), topography devices and OCTs; sometimes the patients watch an informational video on the procedure they are about to undergo, and the ophthalmologist reviews the test results. The ophthal- mologist then proceeds with the eye exam, discussing options with the patient. Finally, surgery is scheduled, the patient consults the financial counselor, and checks out. Reviewing keratometry results with the IOLMaster, Dr. Cohen said technicians should know when to repeat testing. A power difference of 3.0 D or more between eyes, he said, is an indicator, and the first thing the technician must do is repeat the measurements. Dr. Cohen then would compare results, using the results with the two closest readings between eyes. He would also check axial lengths and anterior chamber depth to look at the difference between eyes.
